URBAN REC HOCKEY RELEASE, WAIVER & INDEMNITY NOTE : THIS RELEASE FORM IS A CONTRACT WITH LEGAL CONSEQUENCES. PLEASE READ IT CAREFULLY BEFORE SIGNING.

I want to participate in the 2010/2011 season of Urban Rec Hockey (Urban RecHockey). I understand and acknowledge that the sport of Ice Hockey may involve physical risk. I understand and agree that the games of Ice Hockey are played on surfaces whose condition are variable and unpredictable and in weather, and manmade obstacles which can change without notice, both of which could contribute to the safety of play and result in serious personal injury.
I assume all of the risk of personal injury which may result from participation in activities of the Urban Rec Hockey League, hereafter referred to as the 'URHL'. The player will not hold the URHL, any of the URHL officials, management, owners, nor the proprietor or employees of any ice facility used by the URHL liable for injury which the player may sustain while participating in any league activity.

I understand and agree that I am responsible for any and all medical costs arising from any injuries received while participating in any URHL activities. I certify that I have no known medical condition which would prohibit myself from participating in the sport of ice hockey. The URHL strongly recommends that each player receive a physical check-up before beginning participation in any league event.

I agree to wear a C.S.A. approved protective helmet at all times that I am on or near an ice surface during any URHL activity. This shall include but not be limited to warm-up, breaks in games, while on the bench, and during post game handshakes. I understand that I may be subject to suspension and/or expulsion for violation of this rule. For reasons of safety the league strongly recommends that all players wear an approved full facial mask.

In consideration of Urban Rec Hockey accepting me as a participant during the 2010/2011 season, I for myself, my heirs, executors, administrators, successors and assigns, hereby release, waive and forever discharge Urban RecHockey, its owner and sponsors and all their respective agents, officials, servants, contractors, representatives, directors, elected and appointed officials, successors and assigns of and from all claims, demands, damages, costs, expenses, actions and causes of action, whether in law or in equity, in respect of death, injury, loss or damage to my person or property howsoever caused, arising to or to arise by reason of my participation in the 2010/2011 season of Urban Rec Hockey, whether as a spectator, participant, competitor or otherwise, whether prior to, during, or subsequent to the 2010/2011 season and notwithstanding that same may have been contributed to or occasioned by the negligence of any of the aforesaid. I further hereby undertake to hold and save harmless and agree to indemnify all of the aforesaid from and against any and all liability incurred by any or all of them arising as a result of, or in any way connected, with my participation in the 2010/2011 season of Urban Rec Hockey
